How to Make an Appointment
Call Us
Have your insurance identification information ready before you call or ask about our cash-pay program if you do not have insurance (income eligibility guidelines apply)
Call Bemidji, Halstad or Bagley, Park Rapids, or stop either location during normal business hours
Once scheduled, keep your appointment
Ask about flexible appointments if your schedule is difficult to predict
When you Arrive
Check in at the front desk with our staff
Have your insurance information with you
Be prepared to fill out Medical History, HIPAA and Treatment Consent forms for yourself and each family member who has an appointment
Cancellation Policy: A 24 hour notice is required if you cannot make a scheduled appointment. Because we have so many people waiting to be seen, if you miss three appointments without notice, your scheduling options may be limited.
Who is Eligible
To receive dental care, patients must either:
Be enrolled in a Minnesota Health Care Plan such as Medical Assistance or Minnesota Care. With your member number, you can contact us to make an appointment anytime. We can no longer accept North Dakota Medicaid; North Dakota residents are welcome to inquire about our income-based sliding fee schedule for cash pay. Some commercial dental insurance plans accepted—call to inquire (have your insurance card handy)’
